vBulletin 4.x.x Looking for "Modal Login" box

Hello:

I'm looking for a plugin that will create a modal login box and allow the following customization:

• customizable alerts: something along the lines of "IMPORTANT: Before you sign on, please remember to practice self-care. If a topic gets too heave for you, take a moment to step away and regroup." (basically some sort of message that should be there but not necessarily needs acknowledgment (thing of those notices you see when logging onto computers in a large company)
• multiple mandatory acknowledgments before the user can click the sign on button (e.g. check 1: By checking here, you agree that you have read and understood the <terms of service> (note the terms of service should be a clickable link). check 2: by checking here you agree that you are at least thirteen (13) years old.) you get the idea. If a user doesn't check whatever combination of boxes (e.g. check 2 might be mandatory while check 1 is just a suggestion or whatever configuration the admin wants)
• the above customizatins should allow multiple of them (e.g. more than one sign on alert, whatever number of checkboxes that are set to any requirement level) and allow for text formatting (e.g. bold, underline, font colors, etc. and links)
• fully customizable via phrases and standard vb language and skinning process.

I am trhinking of this for both a corporate intranet site as well as an upcoming project that deals with mental health where having the user check that they meet certtain conditions before "just logging on" would be useful.
