RSS Feed/News Profile images not fully updating

After some users upload new profile avatars, they change in some places like on posts, but not in the profile or the comment box. Those will still use the old avatar. I can't replicate the problem as they seem to fully change for me when I change my profile photo. Any ideas on what is causing this or how to fix it?
